Gather’s Death Certificate lets you quickly capture state-specific information from any device while working with the family. You can even save time by inviting family members to help fill in this information, making the arrangement conference more efficient and ensuring you have the correct information. Once this information is entered, it will be accessible throughout Gather, helping you avoid entering it twice!
Where to Find Your Gather Death Certificate
To access your Gather Death Certificate, open the Vitals page through the percent icon on the left side of the Organize page or through the Death Certificate Task.
The percentage icon will give you a visual of your progress on this specific loved one's death certificate, and the DC section itself will help you keep track of your progress on each section. Information is also saved automatically as you go.

Informant Information
Don't forget your Informant! This section lives on the second page of your Death Certificate under the Approval tab. You can invite your Informant into Gather as a Helper or just store their information to pull to other locations in Gather, i.e., reports and signature packets.

Locking the vital statistics once they have been completed will help keep your funeral directors in the loop about any changes their families need to make to their loved one's information.
Clicking the Approve Answers button at the bottom of the Vitals page will also navigate you to the Informant page.
Death Certificate Overview
Gather allows you to see all the Vital fields at once with the DC Overview. You can use this as a worksheet proof for your families or anywhere else you need to have all the collected data on one form.

Family Helpers
⭐️ Tips and Tricks ⭐️
Invite your families into Gather before the arrangement, and they can begin working on collecting their Loved One's vital statistics even before they come to meet with you. This will give them a head start on collecting the data they need for the death certificate.
Pictured below is what the family helper will see once they've been invited to work on the Death Certificate. If they've been invited into Gather and assigned to work on this task, they can take their time collecting information about their loved one that they might not have on hand during their arrangement.
Once they have completed the task, their answers will be locked from any additional changes. Again, this will help keep your team included in any updates they need to make after already giving the green light on the original information.
